I've been having an unusual problem with the Toolbar Manager. The situation is this:
If I haven't saved I can get it back but if I save it's gone. At first I thought it was just happening on the MDI form but now it's also happening on all the forms. Any ideas?

Could you verify that all references of Infragistics are available in your project and also they exist on the physical path that point them.
Could you let us know if there is something that you have done and you think that may cause this issue, like if you have upgraded your visual studio or install some software or changed the version of the Infragistics components that you are using?

I haven't installed anything recently however there have been security updates applied. Visual Studio was upgraded but that was almost 2 years ago from VS2005 to VS2008. Our OS is XP. We're running Infragistics NetAdvantage 2010 Vol 1. It was upgraded from NetAdvantage 2008 but it was also happening with the previous version.
It only happens when I have the form open in Design mode after running in debug. I try to be careful and close the form designer prior to running in debug but I don't always remember. It's like someone has deleted the control from the screen without any of the other controls being deleted.

Please try the follow:
Close all VS on the machine.
Uninstall Infragistics.
And re install them again.
Open start menu ant go to All Programs->Infragistics->Net Advantage 2010.3-> Windows Form-> Utilities and run “Create Visual Studio Toolbox Tab”
Open Visual Studio and create new Windows Project
Add UltraToolbarsManager and run the project to check if the issue still reproducible.
Could you please tell me what kind of security updates you installed? Is it possible thta this updates causing your issue ?

Sorry I've been unable to take the time to re-install. Our company is getting ready for a refueling outage where my primary role is not in software development. In August we'll be getting new PCs and that should let me know if this is still a problem. I can live with it until then.
