Does anyone know if it is possible to get a checkbox next to the top/root band in the column chooser so that the user can have the option to hide the top level/root band?
I would expect it to functionally work the same as the current checkbox shown next to all child bands.
Is this supported OOTB? If not, what approach would be the best to implement it (other than wrapping the chooser control and adding a separate button to perform the work)?

Agreed, though we have users who have MS Excel muscle memory and it allows you to unselect and then reselect immediately in order to show all columns. although we provide a separate one click mechanism to do this, they still want the functionality this way.
I will go with a custom chooser dialog and have a button or two at the top.

No, there's nothing built-in to allow this. Hiding the root band would hide all of it's child bands, too, anyway, so it really would not make any sense for the user to hide it.
