If you want to just remove a tool instance, select the tool and hit the delete key. If you would like to remove all instance of a tool and the root tool as well, right-click on the toolbars manager component and select 'Customize...'  In the tools tab of the customize dialog, find the tool and hit the delete button under the tool list.
