After going through the steps in the users guide "Creating a Report and Integrating It Into a Silverlight Application', I noticed my Infragistics toolbox tabs had disappeared - both the NetAdvantage 11.1 ASP.NET CLR4x and the Infragistics Reporter tabs.
What is with that?
I ran the toolbox utility to get them back.

I have been looking into your post and I can say that if you have Silverlight 5 installed it is expected and there is an issue submitted to MS forums:
